Mechanism of action: Hexarelin is a synthetic hexapeptide and potent growth hormone secretagogue (GHS) that binds to the ghrelin (GHS-R1a) receptor in the pituitary and hypothalamus, stimulating pulsatile secretion of growth hormone (GH). Hexarelin mimics ghrelin’s action and can synergize with GHRH analogues to further enhance GH release. It also modestly increases plasma IGF-I levels and may influence other hormonal axes, including ACTH and cortisol.
Indications & off-label uses: Initially investigated for treatment of GH deficiency and cardiac dysfunction, Hexarelin was never approved for commercial therapeutic use. It is now primarily used off-label in research and private clinics for purported benefits including increased muscle mass, reduced body fat, improved recovery, and potential cardioprotective effects. Evidence in humans is limited to small studies showing transient GH/IGF-I increases and possible improvements in cardiac function and lipid metabolism.
FDA status & availability: Hexarelin is not FDA-approved for any indication. It is available only through compounding pharmacies.
Safety & monitoring: Adverse effects reported include fatigue, water retention, increased appetite, transient hyperglycemia, and potential desensitization of GH response with prolonged use. Long-term safety data in adults are lacking. As with other GH secretagogues, monitoring for glucose metabolism, lipid profile, and potential pituitary effects is recommended when used off-label.
