Mechanism of action: Sermorelin is a 29-amino-acid synthetic analogue of human growth-hormone-releasing hormone (GHRH) that binds pituitary GHRH receptors and stimulates physiologic, pulsatile release of growth hormone (GH) (and downstream IGF-I).
Indications & off-label uses. Historically, it was used to diagnose and treat GH deficiency in children; more recently clinicians and private clinics have used Sermorelin off-label for adult indications such as improving body composition (increase lean mass, reduce fat), longevity, improved sleep and energy and enhanced athletic performance. However, high-quality, large randomized trials supporting broad anti-aging or performance benefits are limited. Small clinical studies and reviews show some improvements in GH/IGF-I and modest body-composition effects in older adults.
FDA status & availability. Geref® (sermorelin acetate) was previously an FDA-approved product; Sermorelin is now commonly obtained via compounding pharmacies in some markets rather than as an actively marketed FDA-manufactured product. It is not approved by the FDA at this time.
Safety & monitoring. Reported adverse effects include injection-site reactions, headache, water retention, glucose intolerance and other metabolic effects seen with GH axis manipulation; long-term safety data for off-label use in older adults or athletes are limited.
